Pay & Claim Claims Guide

REFUND CLAIMS GUIDE WITHOUT GUARANTEE (PAY & CLAIM)

You are required to fill out a claim form. If not, you can request the form from WHK headquarters or your agent.

You are required to send all required documents, forms and receipts to our Customer Service Center at claim@whk.agency

  1. Claim form 
  2. Hospital or Clinic Original Treatment Invoice/Receipt
  3. Medical report
  4. Doctor's Memo/Referral Letter (Referral Letter)

After sending the documents by email and having received a response from us, you are required to send the documents by post to our head office at D-16-03, Menara Mitraland, Dataran Cascades, No, 13A, Jalan PJU 5/1, Kota Damansara, 47810 Petaling Jaya, Selangor

The process will take up to one (1) to two (2) months from the date the documents, forms and claim receipts are sent.

If the claim is approved after the processing period, you will be reimbursed based on the amount of the claim that has been submitted to us and the insurance/takaful operator.

* All claim application approvals are subject to WHK policy and will be verified by our takaful operator
